Clinical Nurse Specialist

Job Summary
The Clinic Registered Nurse (RN) plays a vital role in providing direct and indirect nursing care to patients at the clinic and in their homes. This position requires close collaboration with the Clinic Physician, clinic staff, and other interdisciplinary team members to manage ongoing care and complex clinical situations, supporting the clinic's primary care operations.
Key Responsibilities
- Conduct thorough nursing assessments at the clinic to determine patient care needs, preferences, and goals, actively participating in meetings to develop comprehensive care plans.
- Deliver nursing interventions through home visits, an integral component of this role.
- Implement and document nursing interventions according to patient care plans, including environment safety maintenance, timely and accurate response to physician orders, medication administration, and conducting ordered tests and treatments.
- Stay vigilant to inputs from team members, patients, and caregivers, promptly informing the interdisciplinary team of any changes in patients' conditions or medical status.
- Collaborate with the Clinic Physician and team for seamless care transitions across different settings.
- Educate patients, their families, and caregivers on disease management, self-care techniques, and preventive measures.
- Assist with the enrollment of prospective patients into the program in coordination with the Marketing Team.
- Engage in end-of-life care coordination and support.
- Provide backup coverage for other nursing roles as necessary.
- Ensure timely and accurate completion of all required documentation.
- Uphold privacy and maintain confidentiality regarding all organizational procedures, and information about employees, patients, and families.
- Adhere to organizational policies and participate in Quality Improvement activities, staff training, and meetings.
- Fulfill other duties as assigned.
Requirements
- Graduate of an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
- Unencumbered California Registered Nurse (RN) License.
- Current Basic Life Support (BLS) / First Aid Certification required.
Preferred Qualifications
- A minimum of two years of nursing experience in a clinical setting, particularly with a frail or elderly population.
- Proficient nursing knowledge and skills for managing frail, elderly patients and complex clinical scenarios.
- Highly motivated, self-directed, capable of task execution in a rapidly changing environment, and adept at making sound decisions in emergency situations.
- Superior clinical, organizational, and communication skills in settings involving seniors, their families, and interdisciplinary team members.
- Proven experience and competency in working with diverse populations.
- Dedication to improving the quality of life for our most vulnerable seniors.
- Bilingual in Spanish and English preferred.
