Job Summary:

The County of Santa Clara is seeking a highly skilled Clinical Nurse Specialist to join our team. As a Clinical Nurse Specialist, you will be responsible for assessing, planning, implementing, and evaluating the nursing care of patients in the pediatric department.

Key Responsibilities:

  • Provide direct nursing care to patients and their families through selected client caseloads.
  • Perform ongoing evaluation of the nursing care of patients requiring continued follow-up.
  • Develop and/or facilitate the development of comprehensive nursing interventions for specific clients based on expert assessment of the client's needs, goals, and outcomes.
  • Collaborate with other members of the healthcare team to develop an integrated plan of care.
  • Provide patient/family supportive counseling and education when appropriate.
  • Coordinate patient care between inpatient and outpatient departments.

Requirements:

  • Graduate of an approved nursing program (preferably BSN), currently licensed as a Registered Nurse (RN) in California.
  • At least five years of experience, evidence of expertise, and competency in the identified nursing specialty.
  • Experience in assessing patient and family learning needs and developing teaching programs.
  • Nationally recognized certification as an expert in the identified nursing specialty preferred.
