RN Clinical Case Manager

Key Responsibilities:
* Assess patient/family clinical status needs related to medical diagnosis, treatment plan, treatment providers, treatment options, and financial resources.
* Collaborate with the patient's medical team and coordinate care including supportive services such as social work, psychology, nutrition, speech pathology, community social support and other specialties.
* Facilitate and secure referrals for homecare, DME, Hospice.
* Intake and schedule all new patients using multiple referral sources such as phone, email, e-consult, and fax ensure all appropriate materials have been obtained including images, pathology, and clinical records.
* Coordinate the scheduling of appointments for new medical and surgical patients, expediting when necessary.
* Maintain referral and navigation logs for all new patients and consult with the appropriate specialty to determine appointment scheduling.
* Maintain current educational materials to be provided for patient, staff, and community members related to specific diagnosis and treatment options relevant to disease process.
* In collaboration with other members of the healthcare team builds partnerships with local agencies and groups that may assist with cancer patient care, support or educational needs.
* Follows patients through cancer treatment and facilitates the transition to survivorship phase of care.
* Coordinate and facilitate tumor board meetings.
* Establishes ongoing dialogue with patient/family to determine if patient's needs are being met physically, emotionally and spiritual focusing on a holistic care approach. Identifies potential barriers to care and facilitate referrals to mitigate barriers.
* Provide cancer screening and prevention education to individuals and groups.
* Promotes awareness of clinical trials to patients, families and care givers.
* Report to cancer committee on the topics of navigation, education, and cancer conferences by tracking and monitoring metrics and outcomes in collaboration with administration.
* Monitors critical aspects of care affecting reimbursement.
* Performs other related duties as required.
