Job Overview

Centria Healthcare is seeking a skilled RN Clinical Supervisor to lead our clinical team in providing exceptional patient care. As a key member of our team, you will be responsible for managing a caseload of clients, supervising field staff, and ensuring compliance with clinical standards and policies.

Key Responsibilities:

  • Perform case opens and assessments to ensure high-quality patient care.
  • Monitor and schedule client visits to ensure timely and effective care.
  • Prepare and process documentation related to patient visits.
  • Communicate with field staff to build relationships and provide support.
  • Review shift notes to obtain recent history on client status and determine the quality of the record.
  • Work closely with the Director of Clinical Services and the Healthcare Team to provide support for staff and clients.
  • Provide a positive experience for staff and clients.
  • Perform Telehealth On-Call duties on a rotating basis.
  • Maintain client care plans, including creating, sending, and tracking new/updated Plans of Care (POCs) and addendums.
  • Provide clinical education to clients, family/guardians, and field staff members.
  • Make administrative and procedural decisions and judgments on sensitive and confidential issues.
  • Document and report on patient progress, challenges, complaints, incidents, and status changes.
  • Manage inventory for supplies and medications on cases when necessary.

Requirements:

  • Bachelor's degree required.
  • Current Registered Nurse (RN) license in good standing.
  • Minimum 1+ year experience in nursing.
  • Knowledge and/or experience in a home care model.
  • Knowledge and/or experience with SCI, TBI, Respiratory failure, Pediatric Populations.

Equipment and Technology Requirements:

  • Working knowledge of laptop/desktop PC.
  • Proficiency in Microsoft Suite (Word, Excel).
  • Proficiency in G Suite (Gmail, Drive, Docs, Sheets, Google Meet).

Other Competency Requirements:

  • Ability to follow written instructions.
  • Ability to use computers and computer/software programs.
  • Ability to communicate expressively and receptively.

Salary Range: $70,000.00 - $75,000.00

Working Conditions:

  • Centria's office hours are Monday through Friday from 8:30 AM – 6:00 PM.
  • Travel is required to perform in-home visits.
  • Additional time or occasional shift in schedule may be required to complete the above work or meet company objectives.
