Job Summary

Wellspring Lutheran Services is seeking a dedicated and compassionate Residential Case Manager to join our team at the New Directions campus in Farmington Hills, MI. As a key member of our multidisciplinary team, you will play a vital role in supporting the well-being and success of children and families in need of our services.

Key Responsibilities
  • Participate in Family Team Meetings to ensure the youth receives an appropriate array of services to stabilize their clinical and behavioral needs.
  • Collaborate with the youth's therapist and service team to meet their therapeutic, psychiatric, educational, medical, and social-emotional learning needs.
  • Develop and implement case plans, write case reports, and maintain accurate records within agency and DHHS time frames and requirements.
  • Support the development and implementation of calming and safety plans for youth in conjunction with the youth, family, staff, and referring worker.
  • Coordinate outside services, including psychological/trauma assessments and auxiliary therapeutic services.
  • Provide direct service to clients, including supervising visits, setting up home passes, practicing coping skills, securing state identification, and collaborating with providers in prospective discharge settings.
  • Conduct crisis counseling as needed with youth and family, promote family inclusion and collaboration in youth treatment.
  • Promote normalcy activities into treatment that complies with the Reasonable and Prudent Parent Standard to help youth and families develop skills essential for positive development.
  • Collaborate with DHS and private agency personnel to develop and implement plans that promote permanency for each resident through return to family or through finding adoptive family/permanent guardianship.
  • Assist with family finding/family search for youth in care and/or assist with development of support for youth for whom there is no identified family.
  • Prepare for and implement comprehensive discharge/aftercare plans, including representation in court as required.
  • Provide aftercare for six months to youth discharged to a community setting, including regular contact with the family and collaboration with local service providers.
  • Respond to after-hours calls that may include emergency hospitalization, emergency notification of families, or crisis intervention as needed.
