Responsibilities:

  • Conduct psychosocial assessments with child and family to identify needs and develop a plan for post-discharge care.
  • Secure community services, such as homecare, medical equipment, and outpatient mental health services, to support patients and families.
  • Provide education, counseling, and resources to help parents cope with their child's illness and develop coping strategies.
  • Identify child abuse and neglect and make reports to the State Central Registry as necessary.
  • Arrange transportation for patients and families as needed.
  • Work with pediatric psychiatry to develop safe discharge plans and obtain psychiatric beds for patients requiring inpatient treatment.

Requirements:

  • Masters Degree in Social Work (MSW) from an accredited program.
  • New York State License as a Licensed Masters Social Worker (LMSW).
  • Experience working with pediatric patients and inpatient social work settings.
  • Strong communication and interpersonal skills.
