Job Summary:

St. Luke's Health System is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team in Twin Falls, Idaho.

About the Role:

As a Home Health RN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement personalized care plans that address the unique needs of each patient.

Responsibilities:

  • Provide comprehensive nursing care to patients with a variety of health conditions, including respiratory, wound care, and medication management.
  • Conduct thorough assessments of patients' physical and psychological needs, and develop care plans to address these needs.
  • Collaborate with patients, families, and caregivers to educate them on disease management, medication use, and self-care techniques.
  • Coordinate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements:

  • Current RN license in the State of Idaho.
  • 1 year of RN experience, with a preference for Home Health or Acute Care experience.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and proof of auto insurance.
