We are in search of a Youth Family Foundations Support Specialist to join our Youth Services team. This role involves providing comprehensive support to young adults and their families participating in our Youth Family Foundations program. Responsibilities include delivering advanced case management, advocacy, and referral services to assist participants in overcoming challenges and achieving long-term housing stability.

Key Responsibilities:

  • Conduct assessments to identify the needs of families or individuals.
  • Facilitate connections to community resources aimed at family development.
  • Enhance access to nutritional assistance.
  • Link families to prenatal services and in-home support.
  • Promote access to educational and employment opportunities.
  • Develop personalized goals with a focus on youth.
  • Help eliminate barriers and navigate obstacles towards stability.
  • Maintain accurate data tracking and reporting in local and state systems.
  • Support existing Youth Service programs and collaborate with team members as needed.

Qualifications:

  • A Bachelor's Degree is preferred, ideally in a Human Services-related field.
  • Two years of experience working with youth and young adults is preferred.
  • Comfortable working with pregnant and parenting youth in both group and individual settings, with experience advocating for families.
  • One year of experience in assessment, advocacy, and case management within a Human Services context is preferred.
  • Strong knowledge of community resources available to low-income families.
  • Exceptional customer service, organizational, and communication skills, both oral and written.
  • Able to manage multiple tasks in a fast-paced environment.
  • Proficient in basic computer software and data collection systems.
  • Strong crisis management abilities.
  • Ability to maintain confidentiality and accurate records.
  • Meticulous attention to detail.
  • A valid Driver's License is required.
