Job Title: Clinical Practice Leader

As a Clinical Practice Leader at Health and Wellness Providers (HWP), you will be responsible for leading all clinical activities at the practice. HWP is a innovative healthcare organization focused on providing chronic care management (CCM) and remote patient monitoring (RPM) services to patients through our novel service offering.

Key Responsibilities:
  • Oversee the clinical activities at the practice
  • Create, update, and maintain clinical protocols for staff members and contracted care providers
  • Work with the team to maintain patient comprehensive care plans, including problem lists, treatment goals, and medication management
  • Provide coaching and guidance to the clinical care team to deliver high-quality care
  • Implement enhancements and improvements in the clinical practice to provide the best care to patients
  • Utilize our specialized CCM/RPM platform and eHR software to document interactions
  • Partner with the patient's care team to drive clinical outcomes, including reducing admissions and hospitalizations
  • Provide medical decision-making for the practice, including direct interventions for patients
  • Participate in continuing quality improvement activities for all staff, including identifying and resolving opportunities for staff education and process improvements
Requirements:
  • Current, valid MA Licensed Certified Nurse Practitioner (NPs) and Certified Nurse Specialists (CNSs) with independent practice authority in MA
  • Willingness to acquire other state licenses, as required by business need
  • 2+ years' experience in a primary care/internal medicine role working with patients with chronic disease states
  • Creativity to drive rapid change and improvement in HWP's service offering to patients by providing high-quality care and care team enablement
  • Experience with delivering care to patients virtually preferred
