About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Care Manager. As a Care Manager, you will play a vital role in providing high-quality patient care and support to our clients in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop strategies to address any identified gaps in care.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Document patient progress and outcomes in a timely and accurate manner, and communicate effectively with patients, families, and healthcare teams.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
