Job Summary

The Ambulatory Triage Registered Nurse is a critical member of the Family Medicine Care team at Community Health Network. As a hybrid RN, you will work onsite in our 96th Street office and remotely from home, conducting phone triage, patient education, scheduling appointments, prior authorizations, prescription refill, referrals, care coordination, and other related tasks.

About This Role

">

We are seeking a highly skilled and compassionate RN with three or more years of nursing experience, preferably in medical office/outpatient settings. Previous phone triage experience is a plus, as well as EPIC experience. If you possess excellent communication skills, team skills, and a caring demeanor, we encourage you to apply.

Key Responsibilities

- Conduct phone triage and patient education

">

- Schedule appointments and manage prior authorizations

">

- Coordinate care and referrals

">

- Perform prescription refill and scheduling tasks

Requirements

">

- Graduate of NLN, CCNE, ACEN, or NLN CNEA accredited school of nursing

">

- Valid license to practice as a Registered Nurse in Indiana

">

- Three or more years of nursing experience

">

- Previous medical office/outpatient experience preferred

">

- Phone triage experience a plus

">

Salary Range

">

$65,000 - $85,000 per year, depending on experience.
