Ambulatory Care Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our progressive Ambulatory Care pharmacy team at Community Health Network.
About the RoleThis is a full-time position that offers a competitive salary of $130,000 per year, plus benefits and opportunities for professional growth and development.
As a Clinical Pharmacist in our Ambulatory Care team, you will participate in team-based care under collaborative drug therapy agreements. You will have the opportunity to make medication changes, manage chronic disease states, transition patients from hospital to home, and provide preventive services such as Medicare annual wellness visits.
Key Responsibilities- Provide direct patient care and work closely with physicians and healthcare teams.
- Collaborate with patients to overcome medication adherence issues and assist them in obtaining cost-effective medications.
- Serve as a drug information resource for other providers and office staff.
- Monitor for drug-therapy-related adverse events and perform clinical or laboratory tests as needed.
- Engage in population health projects to improve patient outcomes.
- Graduate of an accredited college of pharmacy with a Doctorate degree (PharmD).
- Indiana Pharmacist license.
- Successful completion of a PGY1 Pharmacy Residency and PGY2 residency with Ambulatory Care focus is strongly encouraged or a minimum of three years working under a drug therapy agreement in a clinical setting.
- Board certification as a Board Certified Pharmacotherapy Specialist or Board Certified Ambulatory Care Pharmacist is encouraged and required within two years of accepting the position.
