About the Role

As a Physical Therapist Assistant with CenterWell Home Health, you will play a vital role in providing high-quality patient care in the comfort of their own homes. Our team is dedicated to delivering personalized, comprehensive home care services to patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.

Key Responsibilities
  • Administer physical therapy treatment plans developed by physical therapists
  • Provide non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to patients
  • Accurately document daily, weekly, or monthly records of clients' progress
  • Submit clinical paperwork to the office on time each week
  • Notify physical therapists or Executive Director of any equipment failures, lack of supplies, staff or client changes, and other departmental issues
  • Schedule clients and perform varied office tasks
  • Apply concepts of infection control and universal precautions in performing patient care activities
Requirements
  • Associate degree in Physical Therapy
  • Current state license to practice Physical Therapy
  • Minimum one year experience as a Physical Therapy Assistant; Home Health experience a plus
  • Meets mandatory continuing education requirements of CenterWell Home Health and licensing board
  • Maintains clinical competency in Physical Therapy practice and theory
  • Must read, write, and speak fluent English
