We are seeking a skilled and compassionate Home Health Registered Nurse Care Manager to join our team at AdventHealth. As a key member of our home health care team, you will play a vital role in providing high-quality patient care and coordinating services to meet the unique needs of our patients.
Responsibilities- Coordinate and direct the care of a caseload of home patients, prioritizing their needs and adapting to changing circumstances.
- Assess patients' physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, and cognitive status, as well as their discharge planning needs.
- Implement the plan of care through direct patient care, coordination, delegation, and supervision of the activities of the healthcare team.
- Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to patients with various complex home situations.
- Inform physicians, clinical managers, and other healthcare team members of changes in patients' conditions and needs.
- Facilitate and coordinate interdisciplinary care conferences for complex patients.
- Minimum of one year of relevant clinical RN experience.
- Current Registered Nursing License in the State of Practice.
- Valid Driver's License and current car insurance.
