About the Role:
  • Develop and implement personalized care plans in accordance with patient needs and preferences
  • Conduct home visits to clients in designated geographic territories, providing medical care and support as needed
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures, in accordance with established protocols
  • Monitor clients' conditions, reporting changes to Clinical or Client Services Manager, and follow up with doctors' orders
  • Collaborate with healthcare teams to ensure seamless care coordination and case management
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided

Qualifications:
  • A current license as a Registered Nurse in FL
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
  • Ability to work independently and manage time effectively, with strong interpersonal skills and solid computer skills
