Overview

About the Role

This exciting opportunity will allow you to work collaboratively with our experienced clinical team to provide high-quality care to our patients. You will have the autonomy to practice at the top of your license, making decisions that impact patient outcomes and contributing to the success of our organization.

As an Advanced Practice Clinician, you will be responsible for:

  • Providing direct patient care, including assessments, diagnoses, and treatment plans
  • Developing and implementing individualized care plans that meet the unique needs of each patient
  • Collaborating with other healthcare professionals to ensure seamless transitions of care
  • Maintaining accurate and up-to-date medical records
Requirements

To be considered for this role, you must possess:

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • Current licensure through the Texas Board of Nurse Examiners or Licensed Physician Assistant by the Texas State Medical Board
  • Valid DEA license
  • Experience working with primarily Medicare-aged patient populations
Preferred Qualifications

Bilingual (English/Spanish) fluency and/or experience working in a managed care environment are highly desirable.
