Key Responsibilities:

  • Conduct admissions, manage cases, and perform follow-up skilled nursing visits for home health patients.
  • Deliver ongoing care and case management for each patient, ensuring necessary follow-ups as directed by the Clinical Manager.
  • Administer hands-on care, manage and evaluate care plans, and educate patients in accordance with physician orders, under the supervision of the Clinical Manager.
  • Revise care plans in consultation with physicians based on continuous assessments and regulatory requirements.
  • Coordinate comprehensive care involving various healthcare professionals, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
  • Continuously report on patient care, condition, and progress to the patient's physician and Clinical Manager.
  • Implement care plans collaboratively with patients and families to facilitate optimal resolutions of their needs.
  • Oversee and supervise the work of Home Health Aides and Personal Care Workers, providing written care instructions that reflect the current care plan.
  • Ensure accurate documentation completion by home health aides and personal care workers as part of supervisory responsibilities.
  • Prepare essential medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan modifications.
  • Compile visit reports, summarize patient records, and collaborate with other healthcare disciplines to ensure optimal patient care.

Qualifications:

  • Diploma, Associate, or Bachelor Degree in Nursing.
  • A minimum of one year of nursing experience is preferred.
  • Strong background in medical-surgical, ICU, ER, or acute care settings.
  • Experience in Home Health is advantageous.
