Job Summary

We are seeking a highly skilled RN Case Manager to join our Inpatient Rehabilitation team at Bon Secours Mercy Health. As a key member of our team, you will be responsible for coordinating admissions and referrals to our Acute Inpatient Rehabilitation Unit.

Responsibilities
  • Coordinate admissions and referrals to the Acute Inpatient Rehabilitation Unit
  • Collaborate with internal and external referral sources to ensure effective communication and maximum referral opportunities
  • Develop and maintain effective working relationships with patients, families, and healthcare professionals
  • Assess patient charts to determine appropriateness for Inpatient Rehabilitation Unit admission
  • Provide excellent written and oral communication skills to ensure seamless communication with patients, families, and healthcare professionals
Requirements
  • Licensed RN in the State of Ohio, BSN preferred
  • Three to five years of experience in nursing, with two to three years in Acute Inpatient Rehabilitation Facility or Inpatient Rehabilitation nursing
  • Knowledge of reimbursement for inpatient rehabilitation facilities and experience working with third-party payors
  • Ability to pass a FIM assessment with written materials
  • Strong problem-solving skills, good judgment, and initiative
  • Knowledge of current CMS admissions rules, regulations, and policies
  • Knowledge of CARF and Joint Commission standards and regulations for accreditation
  • Working knowledge of various insurance carriers
Working Conditions

This is a PRN position, and you will work in a normal office environment with little exposure to noise, dust, temperature, and the like.

You will be responsible for on-site evaluation and completion of preadmission assessment forms, beginning orientation of patients and families, communication with discharge planners, and physicians.

Collaborate with Rehab Admissions Coordinator and assist in case management for those on the unit.

Maintain contact with internal and external referral sources to assure good communication and maximum referral opportunities.
