Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality patient care in a home setting. As a Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care for patients with various medical conditions.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document all aspects of patient care, including progress notes, treatment plans, and discharge summaries.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Physical Therapist in the state of Ohio.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
