Nurse Coordinator

We are looking for an experienced Nurse Coordinator - Palliative Care to join our team at North Oaks Medical Center. As a nurse coordinator, you will play a critical role in ensuring that patients receive high-quality, patient-centered care throughout their treatment journey.
Key Responsibilities:
- Coordinate patient care by communicating effectively with healthcare teams and stakeholders.
- Develop and implement care plans that address patients' physical, emotional, and spiritual needs.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and optimize patient outcomes.
- Provide education and support to patients and families on palliative care, hospice services, and end-of-life care options.
- Manage patient records and reports, including assessments, care plans, and outcomes data.
Requirements:
- Current licensure as a Registered Nurse (RN) in Louisiana.
- Minimum 3 years of experience in nursing, preferably in oncology, hospice, or palliative care.
- Palliative Care Certification or willingness to obtain within 18 months.
Compensation: The estimated salary for this position is $80,000-$105,000 per year, depending on experience and qualifications.
