Community Partners in Action (CPA) is seeking a highly motivated and experienced Case Manager - Reentry Specialist to join our team at the Hartford Reentry Welcome Center. As a key member of our reentry team, you will play a critical role in supporting individuals returning home from prison to Hartford and the surrounding areas with their successful reintegration back into their communities.

Job Summary:

The Case Manager - Reentry Specialist will provide strategic case management services and assist participants with accessing the community resources needed post-release. This includes but is not limited to:

  • Living wage and long-term employment assistance
  • Access to housing
  • Access to primary care and substance use and mental health services
  • Access to transportation
  • Gender responsive reentry and recovery programs
Responsibilities:

The Case Manager - Reentry Specialist will be responsible for:

  • Providing long-term case management services to participants for at least up to 12 months in the community
  • Working in collaboration with the programs Reentry Navigators
  • Assisting each participant to increase their knowledge and understanding of behavioral patterns in order to promote change and help end the cycle of recidivism
  • Conducting participant risk/needs assessments using motivational enhancement strategies
  • Utilizing the outputs of the assessments to formulate participants Individual Service Plans (ISPs)
  • Making appropriate participant referrals based on individual needs
  • Collaborating with participants parole or probation officers to ensure post-release compliance
Requirements:

To be successful in this role, you will need:

  • Bachelors, preferably in Criminal Justice or Social Services and at least two years of experience providing case management services
  • Strong emotional intelligence and written and verbal communication skills
  • Knowledge of community providers that offer services to the criminal justice population
  • Ability to interact with a multi-cultural population and crisis management skills
  • Decision-making and knowledge of cognitive-behavioral approaches
  • A valid drivers license and reliable transportation
  • Bi-lingual capacity (English/Spanish) preferred
  • A candidate in recovery for at least five years and/or formerly incarcerated is desirable
