Key Responsibilities:

  • Conduct assessments and develop individualized care plans to address resident needs.
  • Monitor adherence to care plans by reviewing documentation and participating in interdisciplinary meetings.
  • Communicate effectively regarding resident conditions with other departments and regulatory bodies.
  • Engage in discharge planning, providing residents and families with information about available programs.
  • Research and recommend improvements to policies and procedures in line with current regulations.
  • Assist in the formulation and management of the department's annual budget.
  • Foster a warm, supportive, and positive environment for residents and staff.

Qualifications:

  • Associate's Degree in Social Work or related field.
  • 2 to 5 years of experience in social services within a healthcare environment preferred.
  • Familiarity with regulations governing healthcare facilities.
  • Understanding of Medicare, Medicaid, and private insurance systems.
  • Strong verbal and written communication skills.
  • Commitment to maintaining confidentiality in accordance with HIPAA regulations.
