At Laurelhurst Operations LLC, we are seeking a skilled Social Services Coordinator to join our team. As a Social Services Coordinator, you will be responsible for planning, organizing, and directing the overall operation of our facility\u2019s Social Services Department.

Key Responsibilities:

  • Develop assessments and written plans of care to identify the needs of each resident.
  • Ensure that all components of the care plan are being followed by reviewing MDS\u2019s and nurses\u2019 notes and attending meetings.
  • Effectively communicate information concerning a patient\u2019s condition with other departments and government agencies.
  • Participate in the discharge planning, providing information to residents and families of programs available.
  • Seek out new methods and make recommendations on changes in policies and procedures based on current federal and state regulations.
  • Assist in preparing and planning the department\u2019s annual budget, and maintain throughout the year.

Requirements:

  • Associates Degree in Social Work.
  • 2 – 5 years\u2019 experience in social services in a Skilled Nursing or similar health care setting preferred.
  • Knowledgeable of the rules, regulations and guidelines that govern nursing care facilities.
  • Understanding of Medicare, Medicaid, Kaiser and private insurances.
  • Excellent oral and written communication.
  • Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
