Job Title: Social Services Practitioner

Adventist Health is seeking a skilled Social Services Practitioner to join our team. As a key member of our healthcare team, you will provide psychosocial assessments, diagnosis, and treatment to patients and families, helping them cope with hospitalization, illness, and life situations.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify emotional, social, and environmental strengths and problems.
  • Develop and implement individualized treatment plans using evidence-based clinical social work interventions.
  • Provide crisis intervention, supportive counseling, and referrals to outpatient resources as needed.
  • Collaborate with multidisciplinary healthcare teams to provide leadership and represent the clinical social work perspective.
  • Document assessment plans, interactions, and interventions according to departmental and hospital guidelines.
  • Provide clinical and administrative supervision to social workers and clinical social workers on the team.
Requirements:
  • Master's Degree in Social Work (MSW) or related field.
  • 5+ years of post-master's experience in healthcare or social service agency.
  • Licensed Clinical Social Worker (LCSW) in state of practice.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
  • Valid Driver's License and must be at least 21 years of age or older.
