Job Summary:
Carlsbad Lifehouse, Inc.

is seeking a dedicated Support Worker to join our team in providing assistance to individuals facing substance use and mental health challenges.

As a Support Worker, you will play a crucial role in offering support, guidance, and encouragement to clients within our various programs, ensuring their safety, well-being, and progress towards recovery.


Salary:
$40,000 per year

Qualifications include but are not limited to:
A Community Support Worker (CSW) must be at least 18 years of age.
A CSW must also either hold a bachelor's degree in a human services field from an accredited university and have one year of relevant experience with the target population, or
Hold an associates degree and a minimum of two years of experience working with the target population, or
Hold an associates degree in approved curriculum in behavioral health coaching; no experience necessary, or

Have a high school diploma or equivalent (GED) and a minimum of three years of experience working with the target population, or hold a certification from the New Mexico credentialing board for behavioral health professionals as a Certified Peer Support Worker (CPSW) or as a Certified Peer Family Specialist (CPFS).

Minimum staff qualifications for Certified Peer Support Worker (CPSW):

Shall be a minimum of 18 years of age;
Shall have a minimum of high school diploma or GED;
Shall be self-identified as a current or former consumer of mental health or substance abuse services and have at least one year of mental health or substance abuse recovery; and LifeHouse
Shall have received certification as CPS.

Minimum staff qualifications for Certified Family Support Worker (CFSW):

Shall be a minimum of 18 years of age;
Shall have a minimum of high school diploma or GED;
Shall have received certification as a CFS.

Have personal experience navigating any of the child/family-servicing systems or advocating for family members who are involved with the child/family behavioral health systems.

CFSW shall have personal experience navigating any of the child-family serving systems or advocating for family members who are involved with the behavioral health systems; shall have an understanding of how these systems operate in New Mexico;
If the individual is a current or former consumer, he shall be well-grounded in his symptom self-management; and
A CFSW must have received certification as a CFSW or (h) CPSW with a family support specialty

Screening Duties:
Complete phone screenings as scheduled.
Complete thorough and accurate documentation of client answers on the screening documentation.
Participate in morning meetings as scheduled.
Assist admissions coordinator in determining appropriateness of fit for potential clients.
Answering and assisting with phone calls in a professional manner.
Sets appropriate, healthy, and therapeutic boundaries with clients.
Coordinate and complete follow-up calls as instructed by clinical director and provide appropriate documentation of results.
Conducting Education Groups as scheduled.
Any other activities as requested by the Clinical Director.

Case Management in Residential Programs:


Complete initial drug and alcohol screening, admissions paperwork, and GPRA at admission and communicate to key staff members any concerns and/or special needs.

Coordinate client orientation process for each new admit including but not limited to checking client into his/her room, inventory of client's belongings, taking photo of new clients and uploading to their electronic chart.

Ensure all appropriate Consent to Release Information for s are filled out correctly, signed, and posted on record.

Encourage client's family/support systems to be involved in community recovery resources, continued care (outpatient, medication management, etc.) with mental health agencies in their community.

Develop a discharge/aftercare plan through engagement of at least two discharge planning sessions with each client. This includes recommendations and applications for transitional and sober living programs.
Ensure transition to outpatient behavioral health treatment in the community in which the client intends to go, link clients to medical providers in those communities (this includes providing required application documents to these services)
Provide guidance and linkages to natural supports during the transition to community life.

Engage in case management meetings with clients, as needed, post-discharge through utilization of a variety of methods to include, but not limited to, face-to-face, internet, and phone calls.

Assist clients to develop and enhance interpersonal, community coping, and functional skills.
Connect clients to safe, stable, and affordable housing that can be sustained over time.

Act as a liaison between the program and community resources to include but not limited to: legal/judicial system, schools, childcare centers, community-based recovery programs, psychiatric services, healthcare, health insurance, income support, etc.

Assess the need for and assist clients in accessing resources from Federal, State, and local programs such as Medicaid, TANF, food stamps, child support, GED, affordable housing, child development services, etc.

Assist clients in scheduling appointments when necessary

Providing educational groups, resource education groups, and other groups that emphasize the need for aftercare services as well as options for such services.

Contact referral sources in cases of unsuccessful discharge or incomplete discharge.
Complete discharge plan with provider/agency names, addresses, phone numbers, date, and time of appointments.

Participate in the treatment planning process with each assigned client by identifying case management and discharge needs as well as documentation of those needs and related goals/objectives.

Participate in Community Group sessions, as needed, in order to assist consumers in presenting their weekly assignments, guide them in conflict resolution, and address their needs.

Conducting education groups as scheduled.
Provide additional group, case management, orientation, and discharge planning coverage as needed, when those service providers are unavailable.
Collaborate with other Clinical Staff to resolve daily issues as needed.
Coordinate transportation for customers to scheduled appointments and emergency room visits.

Make decisions in conjunction with Education Coordinator and/or nursing staff, regarding the need for emergency care for clients, when necessary.

Collaborate with Clinical Director on quality improvement issues as they arise.
Schedule guest speakers relating to discharge/case management needs (I.e. DVR, Oxford House, Rapid Results HIV/HepC Testing.)
Create new forms for documentation of and/or screening for various services as needed.
