Job Title: RN Regional Manager of Care Management

About the Role:

AIDS Healthcare Foundation is seeking a highly skilled RN Regional Manager of Care Management to join our team. As a key member of our Care Management Division, you will be responsible for providing leadership and guidance to our Care Management team, ensuring that our clients receive the highest level of care and support.

Key Responsibilities:

  • Provide clinical guidance and support to the Care Management team, ensuring that they have the necessary tools and resources to deliver high-quality care.
  • Monitor and evaluate the effectiveness of our Care Management program, identifying areas for improvement and implementing changes as needed.
  • Collaborate with our healthcare center team to ensure that our clients receive comprehensive and coordinated care.
  • Develop and implement policies and procedures to ensure compliance with regulatory requirements and AHF's core values.

Requirements:

  • Current RN license
  • 2 years of management experience
  • 2 years of case management experience
