Public Health Specialist

We are seeking a highly motivated and dedicated Public Health AmeriCorps Member to join our team at CHRISTUS Health. As a Community Health Worker II, you will play a vital role in addressing public health needs in local communities and advancing more equitable health outcomes for underserved populations.
Key Responsibilities- Mentor and provide technical assistance to PHACHW I members
- Review and consolidate data and information to be shared with leadership
- Provide screening, referral, and navigation services to connect individuals and their families to social and medical services
- Develop care plans to improve areas of need, providing culturally competent patient-centered care
- Act as an intermediary for individuals, aiding communication between individuals and clinical and community service providers
- Collect and maintain data on referrals and outcomes, adhering to established protocols and protecting patient confidentiality
This is a full-time position that requires a minimum one-year term of service. As a Public Health AmeriCorps Member, you will receive a living allowance, health insurance, and an end-of-service education award. You will not be considered an employee of CHRISTUS Health.
Requirements- High School Diploma or equivalent
- Basic computer knowledge
- Good judgment, initiative, and flexibility
- Ability to develop positive relationships with others, remaining open-minded and non-judgmental
- Strong organizational and communication skills
- Commitment to serving vulnerable populations
- Knowledge or willingness to learn about health issues, the healthcare system, and community services
- Bilingual (English/Spanish) preferred
- 2 or more years of experience working in the community, or working in a healthcare facility, public health department, or community-based organization preferred
- Promotor(a) or Community Health Worker Certification or willingness to obtain within six months of start
Work schedule and type will be determined at a later time.
This is a full-time position.
