Job Summary

We are seeking a highly motivated and compassionate Public Health AmeriCorps Community HEART Work Member I to join our team. As a key member of our community health team, you will work collaboratively with healthcare professionals, community health workers, and administrative staff to provide essential services to individuals and families in need.

Key Responsibilities
  • Provide screening, referral, and navigation services to connect individuals and their families to social and medical services.
  • Develop culturally competent patient-centered care plans to address individual barriers and goals.
  • Act as an intermediary for individuals, facilitating communication between them and clinical and community service providers.
  • Provide education and information about health, wellness, and social services.
  • Collect and maintain data on referrals and outcomes, adhering to established protocols and protecting patient confidentiality.
About the Role

This is a full-time position that requires a minimum one-year commitment. As a Public Health AmeriCorps Member, you will receive a living allowance, health insurance, and an end-of-service education award. You will not be considered an employee of CHRISTUS Health.

Requirements
  • High School Diploma or equivalent.
  • Basic computer knowledge.
  • Good judgment, initiative, and flexibility.
  • Ability to develop positive relationships with others, remaining open-minded and nonjudgmental.
  • Strong organizational and communication skills.
  • Commitment to serving vulnerable populations.
  • Bilingual (English/Spanish) preferred.
  • 1 year of experience volunteering, working in the community, or working in a healthcare facility, public health department, or community-based organization preferred.
  • Promotor(a) or Community Health Workers Certification or willingness to obtain within six months of start.
