As a key member of our community health team, you will work collaboratively with healthcare professionals, community leaders, and individuals to promote health equity and improve health outcomes in underserved communities.
Key Responsibilities- Provide technical assistance and mentorship to Public Health AmeriCorps Community Health Worker I members
- Conduct screenings, referrals, and navigation services to connect individuals and families to social and medical services
- Develop and implement culturally competent care plans to address individual barriers and goals
- Act as an intermediary between individuals and clinical and community service providers
- Collect and maintain data on referrals and outcomes while adhering to established protocols and protecting patient confidentiality
- High School Diploma or equivalent
- Basic computer knowledge
- Good judgment, initiative, and flexibility
- Ability to develop positive relationships with others
- Strong organizational and communication skills
- Commitment to serving vulnerable populations
- Bilingual (English/Spanish) preferred
- 2 or more years of experience working in the community or healthcare field preferred
Full-time position with a TBD work schedule
