Job Summary
We are seeking a skilled Social Services Case Manager to join our team. In this role, you will provide support and social services to health plan members and their families, working closely with interdisciplinary teams to develop, monitor, and evaluate care plans.
Key Responsibilities
- Conduct in-home and telephonic assessments of social service needs.
- Develop and implement personalized care plans to address barriers to healthcare.
- Prioritize cultural and spiritual sensitivity when interacting with patients, families, and caregivers.
- Maintain accurate electronic medical records.
Requirements
- Degree in Social Services or related field, or equivalent experience.
- Ongoing training in social services to stay up-to-date with community resources and educational opportunities.
- Current BLS/CPR certification, valid driver's license, and liability insurance required.
Salary and Benefits
This is an excellent opportunity to grow your career while enjoying a competitive salary of $65,000 - $75,000 per year, depending on experience. Additionally, benefits include employer-paid medical, dental, and vision premiums, employer-matched 401(k), and 15 days of combined sick/vacation time, plus 9 days of holiday during your first year.
