Job Overview

As a Community Health Worker, you will be responsible for enhancing patient management and promoting continuity of care through therapeutic interaction, supportive education, and advocacy.

The ideal candidate will possess excellent communication skills, be highly organized, and able to prioritize their workload effectively. They will also be comfortable driving in the field up to 80% of the time and have a valid Arizona driver's license with liability insurance.

This is an in-field position, requiring strong customer service skills with exceptional attention to detail. The successful candidate will be proficient with Microsoft Office and have previous experience using EMR systems, although this is not mandatory.

Benefits include employer-paid medical, dental, and vision premiums, as well as short-term disability and life insurance. A comprehensive 401(k) plan with employer matching is also available. The company offers 15 days annual combined sick/vacation and 9 days of holiday during the first year, plus your birthday off.

Key Responsibilities:
  • Conduct outreach efforts to engage with members and provide services to best support their overall health.
  • Assist members in their homes, community, or clinic setting.
  • Communicate the purposes of our programs and the impact on member well-being.
  • Document all member encounters and activities, care, and service plans, and outcomes achieved by the member.
  • Educate members on emergency room usage and provide information on viable alternatives.
  • Coach members in chronic health condition management and self-care.
  • Assist members in understanding care plans and instructions from medical professionals.
  • Motivate and encourage patients/members to be active and engaged participants in their health and wellbeing.
  • Advocate for members in any medical and/or behavioral health setting.
  • Assist members in accessing health-related services.
