At Albuquerque Health Care for the Homeless, we are dedicated to providing quality healthcare services to individuals experiencing homelessness. As a Community Health Worker, you will play a vital role in helping our clients navigate care systems, promoting health education, disease prevention, and wellness self-management.

The ideal candidate will have experience working with social services, behavioral health, or healthcare settings, and possess a deep understanding of the population we serve. You will be passionate about individual and community advocacy, skilled in communicating with interdisciplinary care teams, and able to develop quick rapport with our clients.

We offer a competitive salary of $18.27-$19.00 hourly, as well as a comprehensive benefits package that includes low-cost medical, vision, and dental insurance, life insurance, long-term disability, paid time off, catastrophic sick time accrual, 7 paid holidays, flexible spending accounts, and a 401k with employer match.

Key Responsibilities
  • Assist clients on campus and in outreach settings, helping them access public benefits, obtain identity documents, and apply for permanent and transitional housing programs.
  • Facilitate short-term care coordination and linkage to long-term care services for clients with complex health conditions.
  • Utilize community-based tools and methods to assess needs, identify resources, and advocate for systems change with community stakeholders.
Requirements
  • Experience in social services, behavioral health, or healthcare setting.
  • NM Community Health Worker Certification (or ability to complete within first year).
  • Valid driving license with clean record.
  • Ability to work with large, multi-disciplinary teams.
  • High school diploma or equivalent.
Preferred Qualifications
  • Previous work with an EMR.
  • Associate degree in Human Services or relevant field.
  • 1 year experience as CHW or Patient Care Coordinator.
  • Experience with underserved, vulnerable populations.
  • Bilingual Spanish/English.
