Key Responsibilities- Assess and monitor resources to ensure clients live in a stable and safe environment
- Develop and implement individualized service plans based on Recovery Model principles
- Provide case management duties for consumers involved in Behavioral Health Services
- Assist consumers in obtaining and maintaining basic needs, such as housing, food, healthcare, employment, and socialization
- Monitor consumer participation in recovery plans and support services
- Provide effective crisis assessment and intervention as needed
- Bachelor's degree in a relevant field, or equivalent experience
- 2 years of experience in public or private human services, with 1 year in direct client contact
- Clearances: Pennsylvania Child Abuse Clearance, Criminal and fingerprint-based federal criminal history, Valid driver's license
