RN Health Services Coordinator

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Health Services Coordinator. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality healthcare services for adults with intellectual and developmental disabilities.
Responsibilities:- Provide health education and guidance to clients, families, and staff
- Collaborate with the healthcare team to develop and implement individualized care plans
- Conduct assessments and evaluations to identify health needs and develop strategies for improvement
- Communicate effectively with physicians, families, and other stakeholders to ensure seamless care coordination
- Participate in resident meetings and provide health-related updates as needed
- Develop and maintain accurate and comprehensive health records
- Collaborate with the treatment team to ensure the well-being and safety of clients
- Perform basic patient care and treatments as needed
- Measure and record vital signs, administer medications, and maintain electronic health records
- Provide training and education to staff on health-related topics
- Complete audits and ensure compliance with regulatory requirements
- Implement necessary diet changes and ensure physical, medical, and immunization requirements are met
- Full-time Monday through Friday, 8am to 4pm schedule with flexible start times
- Competitive pay range of $40-$45.60 per hour
- Full benefits after 30 days of employment

Qualifications:- Current, valid Registered Nurse license
- Eligible to practice in PA
- Healthcare facility nursing experience
- Experience working with adults with intellectual/developmental disabilities, mental health diagnoses, and/or TBI preferred
- Thorough knowledge of regulatory requirements
- Minimum 21 years of age
- Valid driver's license
