Home Health Risk Assessment Specialist

Job OverviewAs a Nurse Practitioner - PRN/Part Time - Home Risk Assessments with Matrix Medical Network, you will have the opportunity to leverage your expertise and compassion to make a direct impact on the health and well-being of others. You will work collaboratively with physicians, case managers, social workers, family members, and key caregivers to conduct comprehensive health assessments, provide education, and facilitate care coordination.
Key Responsibilities- Conduct adult and geriatric assessments, including medical history, diagnosis, and treatment, as well as provide health education and facilitate referrals.
- Work collaboratively with healthcare teams to ensure seamless care coordination and communication.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
- Collaborate with primary care physicians to provide patient education and follow-up care.
- Master's degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health record software.
- Excellent verbal and written communication skills.
We offer a competitive compensation package, including a salary range of $90.00 to $95.00 per completed assessment (dependent on county), mileage reimbursement, and the opportunity to participate in a 401(k) plan. This is a PRN/Part Time position with flexible scheduling, allowing you to balance your work and personal life.
