IDD Provider Specialist

The IDD Provider Specialist - PRN position is a part-time opportunity that requires a skilled and compassionate individual to provide support services to individuals with intellectual and developmental disabilities. As a member of our interdisciplinary team, you will be responsible for implementing and evaluating therapeutic programs, providing direct care services, and collaborating with family members and care coordinators.
Key Responsibilities:- Develop and implement individualized plans to support individuals with intellectual and developmental disabilities
- Provide direct care services, including assistance with daily living activities, personal hygiene, and medication management
- Collaborate with interdisciplinary teams to ensure comprehensive care and support
- Communicate effectively with individuals, family members, and care coordinators to ensure seamless care coordination
- Monitor and report on individual progress, making adjustments to plans as needed
- High school diploma or equivalent required; degree in a related field preferred
- Experience working with individuals with intellectual and developmental disabilities
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills and proficiency in MS Office
This is a PRN (as needed) position, requiring a flexible schedule to meet the needs of our clients and programs.
