About the Role:

The successful candidate will be responsible for providing high-quality speech therapy services to patients in a home health setting. This includes developing individualized care plans, delivering therapy sessions, documenting patient progress, and collaborating with other healthcare professionals.

Key Responsibilities:

  • Create and update individualized patient care plans addressing specific speech and language needs
  • Deliver speech therapy services and activities for patients with speech and language disorders
  • Maintain accurate and timely documentation of patient records and clinical notes
  • Collaborate with local teams to ensure well-coordinated approach to patient care

Requirements:

To be successful in this role, you will need a Master's degree in Speech-Language Pathology from an accredited program, state licensure as a Speech-Language Pathologist in Texas, and certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
