POSITION SUMMARY: The Community Care Navigator plays a vital role in assisting residents of Nursing Facilities for Mental Health throughout Kansas who are eager to reintegrate into the community. This position employs a person-centered approach to ensure comprehensive care coordination, working closely with residents, their guardians, NFMH Liaisons, Community Mental Health Centers (CMHCs), Managed Care Organizations (MCOs), and the Kansas Department for Aging and Disability Services (KDADS) to deliver holistic support for clients interested in transitioning from a Nursing Facility for Mental Health.
KEY RESPONSIBILITIES:
- Deliver services aimed at achieving program objectives, managing a caseload of approximately 30 clients.
- Ensure timely completion and accuracy of various reports, including mileage and timecard submissions.
- Maintain precise and prompt documentation of service delivery, crafting individualized case notes that reflect appropriate interventions and progress towards client goals.
- Facilitate the creation of a Person-Centered Care Plan in collaboration with the client, family/caregiver, and CMHC NFMH Liaison, utilizing the Preadmission Screening and Resident Review (PASRR) process.
- Conduct periodic reviews of Care and WRAP plans to assess client progress.
- Develop a crisis management plan with clients using the WRAP framework.
- Monitor client status and provide necessary support in alignment with their needs, promptly notifying relevant parties of any changes.
- Coordinate additional services to meet identified client goals based on their unique requirements and functioning levels.
- Facilitate discharge planning in collaboration with clients, MCOs, CMHCs, NFMH, KDADS, and caregivers/guardians.
- Participate in all departmental and agency meetings, trainings, and other required activities.
ADDITIONAL RESPONSIBILITIES:
- Maintain a satisfactory attendance record, providing appropriate notifications for absences and ensuring coverage of responsibilities.
- Demonstrate a competent level of technical knowledge relevant to the position.
- Collaborate effectively within a team, keeping colleagues informed and fostering a respectful work environment.
- Exhibit the ability to assess family needs and maintain professional communication with peers, clients, and supervisors.
- Adapt to new assignments and procedural changes with a proactive approach.
- Perform tasks with a high degree of accuracy and meet established deadlines independently.
- Achieve performance goals as outlined in previous evaluations.
QUALIFICATIONS: Candidates must hold a Bachelor's degree in a behavioral health discipline and possess a minimum of three years of relevant experience. Preference will be given to individuals with experience working with clients who have dual diagnoses of substance use and mental health disorders, or those familiar with NFMHs and/or CMHCs. A valid driver's license and compliance with background checks are required. Frequent travel may be necessary.
