- Collaborate with physicians and other departments to ensure seamless communication and care coordination.
- Patient Care Coordination
- Assess patient needs and develop individualized care plans to meet those needs.
- Coordinate patient care activities with other healthcare team members to ensure efficient and effective care delivery.
- Communication and Collaboration
- Facilitate effective communication between nursing staff, physicians, and other departments to ensure timely and accurate information exchange.
- Build and maintain strong relationships with patients, families, and healthcare team members to ensure a positive and supportive care environment.
- Education and Training
- Provide education and training to nursing staff on best practices, policies, and procedures.
- Develop and implement educational programs to enhance nursing staff knowledge and skills.
- Education and Experience
- Current licensure to practice professional nursing in Tennessee.
- Competent in BLS and/or other specialized life support requirements designated by work area or completion upon hire.
- Healthcare experience required.
- Knowledge and Skills
- Knowledge of scope of the registered nurse, license practical nurse, and nursing assistants.
- Knowledge of and appropriate application of nursing process.
- Knowledge of infection control procedures, safety precautions, techniques involved in administering routine medications and/or treatments to patients.
- Basic computer knowledge.
- Abilities and Qualifications
- Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others.
- Ability to demonstrate emotional maturity by establishing and maintaining effective working relationships.
- Ability to independently seek out resources and work collaboratively.
- Ability to assess nursing needs of acute and chronically ill patients and their families.
- Ability to teach patients and families in accordance with the individualized nursing care plan.
- Ability to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up.
- Ability to use computer and learn new software programs.
- Ability to document and communicate pertinent information using computer and/or paper documentation tools.
- Ability to think critically and problem-solve while remaining focused and organized in a stressful and fast-paced environment.
- Ability to work flexible hours and days to meet facility needs.
