About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a Pediatric Home Health Nurse. As a member of our team, you will have the opportunity to make a profound impact on the lives of children and young adults with specialized healthcare needs.

Responsibilities
  • Deliver high-quality, individualized nursing care to patients based on their unique needs and treatment plans.
  • Assist in the development, review, and revision of the plan of care in collaboration with the healthcare team.
  • Build strong emotional connections with patients and their families to provide a supportive and reassuring presence.
  • Observe, record, and report any changes in the patient's condition to the appropriate personnel and/or physician.
  • Provide ongoing instruction and supervision to patients, families, or other healthcare team members in the plan of treatment.
  • Monitor and record patient condition and document provided care services via electronic charting.
  • Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements
  • Active Licensed Practical Nurse or Registered Nurse License.
  • Problem-solving skills and ability to multi-task.
  • Excellent communication skills.
  • Positive, professional attitude.
