Job Description

Job Summary:

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team at Phoenix Home Care and Hospice. As a Pediatric Home Health Nurse, you will provide high-quality nursing care to children and young adults in the comfort of their own homes.

Responsibilities:

  • Assist in the development, review, and revision of the plan of care for pediatric clients.
  • Provide nursing care in accordance with the physician's plan of treatment and individual plan of care, as authorized by the client's payer source.
  • Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physician.
  • Provide ongoing instruction and supervision of clients, family members, or other health team personnel in the plan of treatment.
  • Monitor and record patient's condition and document provided care services via electronic charting.
  • Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.

Requirements:

  • Active Licensed Practical Nurse or Registered Nurse License.
  • Problem-solving skills and ability to multi-task.
  • Excellent Communication Skills.
  • Positive, Professional attitude.
