We are seeking a highly motivated and detail-oriented Healthcare Outcomes Coordinator to join our Patient Care Coordination Team. In this critical role, you will work closely with patients, healthcare providers, and community organizations to improve health outcomes and quality of life. Your primary responsibilities will include conducting patient outreach, ensuring follow-up appointments, and connecting patients with support services as needed. You will also participate in activities promoting continuous quality improvement using nationally recognized measures such as HEDIS and evidence-based guidelines.

A successful candidate will possess a degree in a medical assisting program or a formal medical services training program from the United States Armed Forces. Alternatively, you may hold a Practical Nursing Degree. A minimum of two years' experience in a clinical practice or other healthcare setting is required. If an LPN, you must hold a current Florida LPN License. If an MA, certification/registration with a recognized organization is necessary. BLS through American Heart Association is mandatory. Additional requirements include physician relations, consensus-building, ability to work independently, and proficiency in Word, Excel, PowerPoint, and EHR navigation.
