About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will work closely with patients, families, and healthcare professionals to provide personalized speech and language therapy services in the comfort of their own homes.

Responsibilities
  • Evaluate and develop individualized treatment plans to address patients' speech and language needs
  • Provide one-on-one therapy sessions to patients, utilizing evidence-based practices and techniques
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
  • Document patient progress and outcomes, providing regular updates to healthcare professionals and families
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements
  • Master's degree in Speech Language Pathology or related field
  • Current certification in Speech Language Pathology (CCC-SLP) or Audiology (CCC-A)
  • Minimum 6 months of experience in speech therapy or related field
  • Home health experience preferred
  • Current CPR certification and valid driver's license
