About Our Home Health Speech Language Pathologist Role

We are seeking a skilled and compassionate Home Health Speech Language Pathologist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in helping our patients achieve their goals and regain their independence.

Key Responsibilities
  • Evaluate and provide speech and language therapy services to patients in their homes or facilities
  • Develop and implement individualized treatment plans to address patients' communication and swallowing disorders
  • Collaborate with our interdisciplinary care team to ensure comprehensive and coordinated care
  • Provide education and training to patients and their families on effective communication strategies and techniques
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Master's degree in Speech-Language Pathology or related field
  • Current certification in Speech-Language Pathology (CCC-SLP) or eligible for certification
  • Minimum of 6 months of experience as a speech therapist or speech language pathologist
  • Home health experience preferred
  • Current and unrestricted license to practice as a speech language pathologist
  • Current CPR certification
  • Good organizational and communication skills
  • A valid driver's license and reliable transportation
