About the Role:

We are seeking a skilled and compassionate Speech Language Pathologist to join our Home Health team. As a Home Health Speech Language Pathologist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:

  • Evaluate and provide speech/language pathology services to patients in their homes or facilities
  • Develop and implement individualized plans of care to address patients' communication and swallowing needs
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination
  • Provide education and training to patients and their families on communication and swallowing strategies
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Master's degree in Speech Language Pathology or related field
  • Current certification in Speech Language Pathology (CCC-SLP) or Audiology (CCC-A)
  • Minimum of 6 months of experience as a Speech Language Pathologist
  • Home health experience preferred
  • Current CPR certification
  • Good organizational and communication skills
