Job Title: LVN Supervisor-Assisted Living-Part Time

Seacrest Village Retirement Communities-Best Place is seeking a highly skilled and experienced LVN Supervisor-Assisted Living-Part Time to join our team. As a key member of our Assisted Living team, you will be responsible for providing exceptional care and support to our residents.

Job Summary:

The LVN Supervisor-Assisted Living-Part Time will be responsible for assisting in the medication management and treatment for residents in accordance with Seacrest Village policies, procedures, and regulations. This role will also involve supervising caregivers, Med Assistants, and med room assistants, as well as providing guidance and support to ensure the highest level of care for our residents.

Responsibilities:
  1. Assist in overseeing resident charts to ensure current documentation and compliance with CA Title 22 regulations.
  2. Conform to regulations set forth in CA Title 22, Department of Social Services.
  3. Assist in documentation and revisions in service plans for residents and CA Title 22, D.S.S.
  4. Participate in assessment and revisions in service plans for residents.
  5. Notify supervisor and A.L. Director of changes in resident's condition and care plan.
  6. Initiate and/or review incidents reports and report to supervisor and A.L. Director.
  7. Respond to emergency calls for residents and non-resident incidents.
  8. Follow regulations and standards of practice related to safety, resident rights, and confidentiality and privacy.
  9. Interact with families in a positive manner and keep families updated on resident's status on an ongoing basis.
  10. Notify A.L. Director when a resident is admitted to the hospital.
  11. Assess and perform wound care treatments as ordered per physician.
  12. Adhere to Universal Precautions and Infection Control procedures.
  13. Set up POS and MAR for new residents and assist the Director as requested during the Admission-re/Admission process.
  14. Supervise and participate in monthly Recap process.
  15. Assist with reporting appropriate documentation to LPA as needed.
  16. Demonstrate proficiency in use of current EMR system/program.
Requirements:
  1. LVN license in State of California.
  2. Minimum one year experience as LVN in similar setting.
  3. Ability to lead/supervise staff reporting to the position.
  4. Current LVN license in State of California.
  5. CPR certification.
  6. CA driver's license.
  7. Demonstrate ability to schedule staff, orient, audit time sheets, supply inventory, and billing services.
  8. Working knowledge of blood pressure equipment, blood glucose, thermometer, and oxygen tank equipment.
  9. Able to adapt to changing demands of position.
  10. Possess and implement appropriate phone etiquette.
  11. Able to pass physical, TB test, and obtain fingerprint clearance as required by our licensure.
  12. Familiar with computer-related skills necessary for assisted living programs and communications.
  13. Able to practice within Seacrest Village policies and procedures and to ensure compliance with same.
  14. Knowledge of pharmacology.
  15. Able to work weekends, holidays, and shifts when necessary.
