Job Summary

Key Responsibilities
  • Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities.
  • Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans to meet their unique requirements.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Monitor and report any changes in residents' conditions, providing timely updates to the healthcare team.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current LVN license in the state of California.
  • Minimum one year of experience in long-term care or a related field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to lift, transfer, and position residents with proper body mechanics and assistive devices.
  • Basic computer skills and knowledge of electronic medical records (EMRs).
