Healthcare Professional with Expert Clinical and Systems Management Knowledge

A skilled Registered Nurse Case Manager is sought to join our team at CAMC Health System. This role requires a strong background in clinical expertise, as well as exceptional communication and leadership skills.
Key Responsibilities:
• Collaborate with healthcare teams to deliver high-quality patient care services, ensuring optimal patient outcomes.
• Identify opportunities for improving patient care processes, implementing evidence-based practices to reduce length of stay and resource utilization.
• Provide expert guidance on health care regulations, ensuring compliance and revenue integrity.
• Evaluate complex patients for needs and barriers to discharge, identifying potential re-admissions and developing strategies to mitigate them.
• Facilitate smooth transitions between facilities, ensuring seamless care coordination.
• Act as a resource for staff, providing education and support on patient care management best practices.
• Safeguard patient rights and maintain confidentiality, adhering to HIPAA guidelines.
• Collaborate with social workers to obtain medication prior authorizations, ensuring timely treatment initiation.
• Analyze trends and report practice issues impacting finances and quality care, driving initiatives to resolve these concerns.
• Direct physicians, residents, and other team members to address system processes hindering timely care delivery, fostering a culture of efficiency and effectiveness.
• Identify educational needs within the multidisciplinary team, designing continuing education programs to enhance knowledge and skills.
• Participate in hospital committees as requested, contributing to continuous quality improvement efforts.
Required Skills and Qualifications:
• A Bachelor's Degree in Nursing or related field is required, with a minimum of 7 years of relevant clinical experience. Substitution: An Associate's Degree with 9 years of experience may be considered in lieu of a Bachelor's degree and 7 years of experience.
• Active licensure as a Registered Nurse is mandatory.
Compensation and Benefits:
This role offers an estimated salary range of $85,000 - $115,000 per year, commensurate with experience. Additionally, benefits include comprehensive health insurance, retirement savings plans, paid time off, and opportunities for professional growth and development.
