Job Title: Licensed Practical Nurse/Licensed Vocational Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse/Licensed Vocational Nurse to join our team at STGi. As a key member of our healthcare team, you will provide high-quality patient care and support to our patients in a fast-paced outpatient clinic setting.

Job Summary:

The Licensed Practical Nurse/Licensed Vocational Nurse will work under the direction of the CBOC Clinic Manager, PACT Provider, and STGi Program Manager to provide a wide range of clinical and administrative nursing care. This includes assessing patient conditions, administering medications, performing various nursing procedures, and documenting patient information in medical records.

Key Responsibilities:
  • Assess patient conditions and develop individualized care plans
  • Administer medications and perform various nursing procedures
  • Document patient information in medical records and VISTA
  • Provide health education and resources to patients and their families
  • Participate in performance improvement initiatives and maintain confidentiality of patient information
  • Assist in maintaining the VISTA clinic reminder tracking system and scheduling patients for appointments
Requirements:
  • Minimum of two years of current clinical experience working in a similar environment
  • Knowledge of basic methods and procedures for coordinating and scheduling clinical care
  • Thorough knowledge of the variations in anatomy and ability to apply practical nursing principles, procedures, and techniques
  • Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations
  • Current and unrestricted nursing license in the state in which the LPN/LVN practices
  • Retain current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) from AHA or MTN
