Regional Variable Site Specialist

As a Regional Variable Site Specialist, you will be responsible for managing patients and coordinating services provided by multiple disciplines within and outside the health system. You will maintain a collaborative practice agreement with specific physicians and exercise independent judgment in the assessment, diagnosis, and initiation of selected protocols/procedures.
You will provide educational support for patients, significant others, health team members, and the community, and coordinate QA/QI studies to measure outcomes of care.

Essential FunctionsYou will function in the role of an advanced practitioner to coordinate comprehensive care delivery. You will write orders/prescriptions for standard formulary medications, laboratory, radiographic studies, and consultations, coordinating the scheduling of admissions, diagnostic evaluations, follow-up exams, laboratory tests, referrals to home care, social services, and other community agencies, etc., to ensure continuity of care and coordination of services.
You will obtain histories and perform physical exams. You will assess the patient's/significant other's, health care team's, school's, and other community educational needs and develop, implement, evaluate, and revise teaching strategies/educational programs related to your area of specialty.
You will ensure that operations are in compliance with professional standards, applicable laws, accreditation, compliance/regulatory requirements, and that organizational goals are being followed.
You will practice in accordance with the Michigan Public Health Code. You will deliver care in a manner that is culturally sensitive and preserves patient autonomy, dignity, and rights. You will act as a patient advocate, crossing multidisciplinary lines to meet patient needs.
You will communicate, collaborate, network with, and act as a consultant to other members of the health care team within and outside Spectrum Health.
You will act as a consult in your area of expertise. You will perform and assist with designated procedures. You will document in the medical record and dictate information as appropriate.
You will provide input into the strategic planning, implementation, and evaluation of service/program. You will measure the outcomes of care and evaluate services across the continuum of inpatient to outpatient services. You will make recommendations to improve the quality of patient care provided and utilize the scientific method to research clinical problems.
QualificationsNurse Practitioner:
Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master's, post-master's, or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
3 years of relevant experience current, relevant clinical experience
Previous experience functioning in a collaborative role as a Nurse Practitioner
LIC-Nurse Practitioner (NP) - State of Michigan
CRT-Basic Life Support (BLS)
CRT-Neonatal Resuscitation Program (NRP)
CRT-Pediatric Adv Life Support (PALS)
Physician Assistant:
Required Master's Degree Graduate of an accredited Physician Assistant educational program
LIC-Physician Assistant - State of Michigan
CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants
CRT-Basic Life Support (BLS)
